This device is apparently quite common in countries where electrical standards are more relaxed and the climate is a bit milder. It's basically a chunky shower head that has a heater built into it. If you do a search for suicide shower on the Internet you'll find various pictures and videos of these things installed badly.
If you do find one of these in your travels then be very aware that there are often badly insulated mains voltage connections in the vicinity of the unit, possibly right next to a metal tap. Some have an earthing/grounding facility, but this may not be attached, or if it is it may not be electrically intact.
If you get too close to the shower head in use you may get an electric shock conducted through the water. This is made a bit more awkward by the fact these units are generally mounted quite low down.
The only safe way to use these is to either ascertain if they have been installed safely or use them to fill a bucket with warm water and then wash yourself down with that. These things are truly worthy of the title deathtrap.
I'm not sure if this particular unit would be usable with an RCD or GFCI since there is a short water-filled path between bare live metalwork and a strategically placed earth wire.

The little hose that comes off the shower head is for rinsing your difficult to reach areas. My landlord says we can't use the warmer setting – it will burn out the shower. The lower the pressure, the hotter it gets.
We used one at a hotel that had a fuse in it somewhere. They had to change it as we had no warm water the first night.
Droplets of water do not carry current if voltage is low (up to about 1kV) but when it's higher then current can jump from drop to drop. This is the difference that you can't electrocute yourself by peeing on the third rail (you only get arrested) but you can get tingle from cattle fence.

I’m in Colombia right now. I’ve seen some of those showerheads till now, none of them earthed. The one in the apartment I am at now is broken, so we may get a new one and I’ll see whether they even have an earthing connection.

You've got all the nasty stuff, just like a knockoff phone charger it's not uncommon to read about someone being electrocuted here in Thailand from an improperly installed shower box. They at least have ground fault switches built into them.
I once discharged a cap over gfci and it popped. Fucking insane
